Enrolment Form and Requirements

Enrolment Form

It is an Education Department requirement that a Birth Certificate be sighted before children may be permitted to attend school.

Please ensure that you have the certificate with you during the enrolment process. Also, please try to have the School Entry copy of the Immunisation Certificate.

(Copies can be obtained by contacting Medicare on 1800 653 809 – Immunisation History Statement.)
